About the Challenge<\/h2><\/span><\/div><\/div><\/div><\/div><\/div>Strategically focused on individuals <\/span>transitioning<\/span> from unhoused to <\/span>housed<\/span> in the Moncton area, <\/span>a<\/span> Rapid Innovation Challenge was launched <\/span>in <\/span>June<\/span> of 2024, <\/span>to <\/span><\/span>explore<\/span><\/span>, <\/span><\/span>develop and test solutions that connect individuals to the essential health and support services they need<\/span><\/span>. This initiative aims to <\/span><\/span>integrate resources from healthcare and community sectors to collaboratively address and resolve the urgent<\/span> and diverse<\/span> needs that are critical for long-term wellbeing and sustainable living situations<\/span><\/span>. <\/span><\/span><\/span><\/p>\n A Systems Leaders group was formed <\/span>representing<\/span> leadership from UNB Nursing, the City of Moncton, GNB, Horizon Health, <\/span>Vitalit\u00e9<\/span>, the Human Development Council, RCMP and the <\/span>Northpine<\/span> Foundation<\/span>.<\/span><\/span><\/span><\/span><\/p>\nChallenge participants formed streams addressing three critical focus areas: addressing stigma to support access to long-term housing and healthcare; a supported workforce – supporting workers who work with those transitioning to housing; and, a connected system – a system that is aligned in addressing the diverse needs of individuals transitioning from homelessness.\u00a0<\/span><\/p>\nPrototypes have been developed and are currently being tested with the help of UNB nursing students. Testing will wrap up in the Spring of 2025.
